For Patients With Insurance:

An insurance information form is available to download and complete. Keep in mind that our records will only be as accurate as the information YOU provide. With today’s technology, our claims are forwarded almost immediately after treatment is complete. If the information form is incomplete, we will be unable to submit a claim and payment will be required at the time of service.

For Patients Without Insurance:

Payment for professional services are due at the time of the appointment. Should there be an extensive treatment plan, special arrangements can be made.
